About Us

Medical Records are used in every aspect of health care planning and delivery. Their primary purpose is to serve as a basis for planning patient care.

A medical record contains a patient's medical history, a physical assessment, test results and notes about the patient's treatment and condition. The medical record serves as a means of communication among all members of the health care team, including physicians, nurses, laboratory technicians, therapists and many others. Information from the medical record is used in continuing medical education, research and health care planning and management. Data from medical records helps health care facilities evaluate the types of services they should offer and project future patient volume and treatment needs.
