In the following a variety of survey forms that the CHE program uses to collect information about the level of achieving the course and program outcomes in addition to educational objectives. The forms are available in PDF and DOC format for convenience. They are also available online. Constituents are expected to download the corresponding survey form, fill it out and email to Accreditation Committee coordinator Prof. Emadadeen M.
